At FUNdamental Pediatric Therapy, we understand that identifying your child's needs and finding excellent therapy can be a challenging time as a parent and/or caregiver. Our knowledgeable and friendly staff are here to help you through the process. We've created the following step by step guide to help you in our thorough evaluation process. We will take the time to evaluate your child comprehensively, as well as educate and provide resources to your family to ensure your child's greatest success at FUndamental Pediatric Therapy.
How to get started

Step 1: Determine A Need
Your child may be recommended for a pediatric speech, occupational, or feeding therapy evaluation directly by your child’s physician OR you may have concerns about your child’s development. Check out our milestones here to see if your child is meeting age-range expectations.
Step 2: referral process
A physician’s referral is required prior to an evaluation being completed. Your child’s physician can fax it to our office at (318) 550-4058.
Step 3: insurance verification and benefits
Prior to your child’s evaluation appointment, our office manager will gather information about your insurance policy (deductible, co-pay, co-insurance, patient responsibility, exclusions, etc.). This gives us information about what is or what is not covered under your plan. Please make sure you have complete understanding of your policy so we can together to make a financial plan if needed.
Step 4: Scheduling and Intake
Our office manager will get you scheduled and provide you with TWO sets of forms to complete prior to your child’s appointment:
1. Patient Intake (paperwork giving us permission to complete the evaluation, HIPAA forms, FUNdamental policies, etc.)
2. Therapy Intake paperwork (birth, medical, developmental information etc. plus an intake of child strengths, weaknesses, and parent concerns we need completed in order to complete a thorough evaluation; links are included below)
Step 5: evaluation
On evaluation day, bring your intake paperwork, any relevant documentation (e.g., medical records, school reports, documentation between any professionals working with your child), and your specific concerns and/or challenges faced.
During the evaluation, your therapist will complete standardized testing (designed to tell us where your child’s skills fall in comparison to children their age), informal observations, and additional informal testing as needed. We will also review the intake paperwork with you, ask any follow up questions, discuss your concerns, and discuss next steps and recommendations.
